Sweem is impotent. Cialis, Levitra, Viagra, Yohimbine, Fo-Ti don't work. When JohnThomas makes an appearance he quickly goes back into hiding, but he does like to rise strong and firm during sleepy time. Sweem goes on making love like a lesbian with many grateful partners but life is still missing an ingredient. Any things out there that'll work for a JohnnyTommy that doesn't respond to all the popular methods.

Well what could be the underlying issues? Is the patient taking any prescription/non-prescription drugs? Circulatory issues? What other health issues are at hand? Is the patient really the 47 years of age they say they are? What has their medical doctor said to them? The patient HAS seen a medical doctor about this issue, right?

The patient never mentioned age, but it's been pretty much a life long issue.
Not related to drugs. Urologists milked the insurance company with their tests and they came up with nothing except saying it's psychological. What's that mean? Thousands of dollars in sex therapy? Sweem ain't rich.
Hearing this , ya can't help thinking there's got to be some kind of drug that could excite johnny, physically or psychologically.

Not trying to accuse or anything but it sounds like the patient is gay? Maybe the patient is just not interested in girls not matter how hard he tries to be. Maybe the patient should be more truthful with him self and really find out whats going on with him self he will figure it out soon enough.

Sorry, but I'm afraid the problem is most likely psychological. Erection occurs during REM sleep, but it is not necessarily related to the content of the dream, it's just a physiological correlate of REM sleep. Since you report that erection occurs just fine during sleep, that rules out physiological impotence and leaves psychological impotence.

This is more in line with performance anxiety. The mind is powerful. When a person allows his mind to work against him, it can be downright destructive.

Or maybe like what Rollin_Raleigh said, the patient should be honest about what really turns him on.

How is the penis working while masturbating? The patient still has troubles or it works like it should be?

Normal erections while masturbating but difficulties while with a partner are a pretty sure sign of performance anxiety.That is, when a guy is too worried about being bad at sex or is stressed out so the boner is lost.

Also good erections while masturbating can indicate that the patient is homosexual or just gets turned on in a different way or by a different thing.Each person is unique and it's nothing shameful about being different and wishing different ways of sexual pleasures as long as both(or w/e number) of partners are consenting.

The invisible purple ninja turtle had and maybe still has performance anxiety.A partner you're comfortable with is a great way to kick the bad habit.He got desperate and tried viagra too (50mg) and it worked.But these ED medicines don't work like in the movies with 4-6h of constant hard on.It's just a lil bit easier to get it up when stimulation is included.Also, side effects do suck.Clogged nose and intense headaches.

There's a condition that involves men losing their erections once they penetrate. I forget the name of it but I think it has to do with a sensitive glans coming into contact against a surface. If this isn't an anxiety problem, a bit of lubricant might work wonders. You could try a few different types of condom too. Ask your doctor.
