hydrocodone 5mg not working for pain, what to try?

SWIM got some sprains and he was prescribed 5mg hydrocodone/500mg APAPs by his doctor. Right now, he's not enjoying the pain.

Not surprisingly, it doesn't touch his pain any better than plain acetaminophen (APAP, paracetamol, Tylenol), which is basically, hardly any effect.

He just started taking Naproxen sodium 220mg (OTC in the U.S.) which lowers the pain to annoying, but bearable level. He's new to this stuff, but he likes it. The 8 hour relief is nice and he responds to it better than Vicodin, but... more relief with minimal affect on his state of mind would be nice.

SWIM has had plenty of experience with hydrocodone since a kid(U.S. tradenames Lortab, Vicodin) as he was handed hydrocodone for every kind of post-dental procedure, pain complaints and post-operative pains.

He's aware that some population lacks the enzyme to convert hydrocodone or oxycodone to their active, -morphone, form, however SWIM doesn't think he's one of them as he feels an annoying buzz from 5mg hydrocodone, nausea from 5mg oxycodone, puking at 10mg oxycodone. It was probably a mistake even getting this prescription filled, because he hasn't had any luck with it, but its been like 2-3 years since he's last had it, so he forgot about it.


hydro is out, b/c of ineffectiveness, oxy is out, b/c of side effects. What should he ask for? He's thinking about trying tramadol. Please advise.

5mg hydrocodone/500mg APAPs is used for moderate to severe pain. If it's just a sprain then elevating the affected limb (if possible) and ice packs would help. Strong opiates aren't usually needed (or particularly useful) for sprains.
Pain is a signal that we feel to enable us not to do further damage. Before taking anything else it might be safer to get it checked out. If it's more serious than a sprain swiy could be just masking the symptoms.

If swiy Doctor is a sport, perhaps he'll humor swiy with some ten mg Norco (10 mg Hydrocodone, 325 APAP), that would permit to try a higher dose of Hydro for the breaklthrough pain without bludgeoning swimself with APAP.

Sprains and soft tissues injuries are seriously business. They can definitely range in moderate to moderate-seve pain range. SWIM is hurting a lot. It's also a major cause of causing professional athletes from not being able to play for weeks and even months.

As SWIM said, he was evaluated by his doctor, was prescribed, hydrocodone. He's even been X-rayed to to check for anything that might be broken. Thankfully there was none, but he's still in a great pain. He's told it's a matter of supportive care.

SWIM could try. He doesn't want to be marked as DSB patient though.
