LSD Production and varities?

Swim was wondering about the process of making LSD.
Swim knows a lot about LSD, but not a lot about how exactly it is made, beside i know from ergot. I am hoping to get answers from people who know from experience.

How does one usually make LSD and with what?

Can LSD be made from LSA, and how?

What is the difference in LSD that makes some good and some bad?
