Safe tramadol combinations that would lower/eliminate the risk of seizure?

SWIM has been taking tramadol recently, but has not combined it with anything. SWIM was wondering as to various drug combinations that would make tramadol safer in terms of the seizure risk it causes. SWIM is aware that some possiblities could involve sedatives, which obviously could be a risk in itself, but SWIM would always test the waters and ensure that reasonable dosages were taken (and, compared to other opioids tramadol seems comparatively safe when it comes to respiratory depression). So any information anyone has on (safe) possible combinations, doses, and how affective they would be would be much appreciated (bearing in mind that the other half of the combination doesn't have to be recreational). If it would help, SWIM ensures that he never takes tramadol more frequently than once every 5 days.

Some specific ideas that could use feedback (though any suggestion at all is welcome):
  • SWIM has some pregabalin, which he understands has a use in treating seizures, as well as having an excellent safety profile.
  • I have heard that pregabalin may be useful for reducing the seizure risks associated with tramadol, but I can't find much reliable information on the subject.
  • Benzodiazepines are used to treat seizures, though this seems like the most risky option so far. In a similar vein to this idea, SWIM has some zopiclone that might be suitable in a similar manner (though SWIM didn't find the recreational effects from zopiclone to be that enjoyable).
Thank you

Re: Safe tramadol combinations that would lower/eliminate the risk of seizure?

A therapeutic dose of a benzodiazepine would probably be the best option. The doses needed to help mitigate any possible seizure risk are going to be very low and thus the CNS depression induced would be negligible. Tramadol doesn't have a very pronounced respiratory depression in itself anyway so I really feel the risk with small amounts of a benzodiazepine would be almost zero. 5mg of diazepam or a similar equipotent dose of another would be sufficient, a similar acting drug like zopiclone should do the trick too.

Thanks for helping, and suggesting the dosages that would be neccesary.

swim prescribed tramadol for my back and swim frequently uses his prescription of xanax on them and it's a very chill high. If you can handle maybe xanax and a kratom combo swim had was awesome.very relaxing and if you smoke a blunt on it just sit back and take your ride lol. Other than that swim wouldnt mix anything.

Re: Safe tramadol combinations that would lower/eliminate the risk of seizure?

Yes, go with a benzo. Diazepam would be your best bet for seizure reduction with clonazepam at a close second. Both are used in the treatment of seizure. Most people find the combination very euphoric as Benzos can potentiate opiates. Pregablin is not what you want in this case.
If SWIY has a problem with seizure, tramadol may not be the opiate for SWIY. Hopefully this helps and you're right, do be careful combining medications.

Oh, and ignore the zopiclone. It will just knock you out or make you feel very messed up if you decide to stay awake through it. It is a hypnotic-sedative and while it acts on the same receptors as benzos do; it does not have the same effect.

Although I've never heard of any problems, I've read from a few sources that clonazepam was serotonergic. So while it would reduce the risk of a seizure, theoretically, it might increase the risk of other symptoms of serotonin toxicity in combination with tramadol.

OP also stated that he uses tramadol no more than once every 5 days. Less than half of diazepam's active metabolite, desmethyldiazepam would be eliminated in that time, so benzo tolerance would inevitably result.

If OP is strict with his tramadol/benzo use, IMO he'd probably benefit most from a shorter acting benzo like alprazolam or lorazepam, both quite potent anticonvulsants as far as I'm aware. 5 days would be long enough for the benzo to fully clear the system and so I think physical dependence would be less likely to develop.

(When I said OP, I of course meant his friend, SWIhonourableone)
