SERVICE DETAILS

The Pathway Project offers a comprehensive structured day-care service for clients who have experienced substance misuse issues and are stable. The service aims to assist the client back into the community and provide help and support for their health, housing, financial management, benefits, employment, training, volunteering, education using community resources and activities.
The project provides an individual tailored programme of support based on assessment. Each client is allocated a key worker. A menu of interventions (as below) are incorporated into a structured care plan based on need in agreement with client, this is used to record progress and milestones and are reviewed weekly. Some of the main interventions are:


· Information and advice about safer drug use.
· Counselling
· Group-work
· Support
· Links with prescribing services.
· Relapse prevention.
· Harm minimisation.
· Housing/benefits advice.
· Mentoring (certificate).
· Basic skills (certificate).
· CLAIT (certificate).
· Football coaching awards.
· Acupuncture.
· Job search/CV's.


WHO IS SERVICE FOR?

Recovering drug users, or ex-drug users, aged 18 or older who are on a drug treatment programme and are relatively stable and seriously trying to reduce their drug use.
The service is not for chaotic users.
The service does not admit those who are intoxicated through drug use, or whose behaviour poses a threat to the well being or recovery of other service users.


REFERRAL

Self referral or referral by any agency.
Referral can be in person, by telephone or in writing.
