Dose - Methadone cross tolerance

Oy! Swim has been using methadone lately. about 35 or so times now. Most of the times have been within the last month and a half. Swims tolerance went up, and quick. swim uses at least 60mg, per dose. at times, around 100mg. Lately, he cant get any methadone. Sence he uses drugs to word off severe depression, untill he can get on anti-depressents, he has been looking to deferent places for help. Help arrived in the form of 57, 4mg hydromorphone pills. Swims absolute favorite drug is hydromorphone. He hasnt taken it in about 2 months. But because of the amount of methadone he took, and methadones cross tolerance, he doesnt know how much he should take. He used to take around 28mg. He would see for himself, but he doesnt like to wast drugs. So, can past methadone use, and tolerance, make you more tolerent to hydromorphone? Thanks in addvance.

As far as SWIM knows, a tolerance to methadone will develop a cross tolerance to any opiate, synthetic or otherwise, simply due to the fact that methadone has the ability to saturate opioid receptors more thoroughly than other opiates. However, hydromorphone/dilaudid was the only synthetic besides fentanyl that SWIM could really ever use with some success while on a daily dose of 130mg/day of methadone. The dose and daily dose consecutiveness of methadone will have a direct effect on tolerance factors for other opiates. Methadone has a long half life (24+hrs) and the half life of dilaudid is much shorter. So this also needs to be taken into account. SWIM would recommend using a lower initial dose of dilaudid to start and just re-dose 45min later if desired effect is not achieved
