Surgery and Heroin use

Swim is scheduled for elective surgery in couple of weeks. It is quite lengthy and swim will be put under.
SWIM is also a heroin user; using bout ¼-½ gram per day. So, SWIM has a few questions...

Should the doctor be informed of usage
Will SWIM have any adverse affect because of usage
SWIM is also taking advantage of this surgery as a good time to quit and go through withdrawls since swim will be all F**cked up from the surgery anyway.

Any help and-or suggestions is very much appreciated.
