Don't answer his calls.
Stop hanging with other user friends.
Tell yourself you can and will do it.
Tell yourself that cocaine is going to kill you one day.
connect with old (clean!) friends and family.
fill the vacuum with something else.

Just have a look at the rules specially the self incrimination one. The list your friend made looks very good to swim.

swim stopped smoking crack by sniffing coke and taper down from there, he has also stopped cold turkey a few times..

Lucky now swim hates crack even if he could get it for free he wouldn't touch it and he doesn't like coke anymore either !

Really hope for your friend he will get there too, all the best !

that is a good list. swim just wants to say that in relation to losing your wife, swiy needs to face the grief head on. it doesnt matter how much drugs swiy takes, it will never go away. on the flipside, swiy can tackle it head on and learn how to live with it/make peace with it.

very sorry to hear about swiy's situation, and swim wishes swiy all the best of luck with his journey.

some other suggestions:

-change phone number.
-change location/career.
-definitely make amends with family/clean mates from the past.
-learn to recognize the triggers, and take action when they arise.

write down every little bad thing it's caused. Wether it be used money for food on crack, or how swiy got robbed, and mention each time, who swiy robbed, friends lost, "friends" gained. write it, look at it, and keep it in your wallet every time you want to hit the pipe again.

Swim had a particularly bad weekend and is ready to start trying to stop. Swim was had for $100 by someone who did the same thing to swim twice before. Swim was left feeling very bad, even sick because of it, not to mention really stupid. Swim told her never to call again.

another important topic is not to substitute cocaine with other drugs. swapping the witch for the bitch never did anybody any favours.

No drinking! Swim tells me that being drunk causes one to make bad decisions.

Remember swiy can get through this. Making the decision to quit is the hardest part, that's the first stage in quitting. Swiy has acknowledged there's a problem, so that part is done, swiy is already getting through this. Just take each day as it comes.
Swiy is doing great.

SWIM may suggest out/in - patient treatment? SWIM dont wanna be preachy or sound taboo, though treatment can be a very beautiful process. To kick crack use using the greatest techniques known to man kind still would be a tumultuous battle. Sometimes, for certain individuals, getting out of their zone, switiching to a healthier enviornment, and focusing some time on their selves can have a truly rewarding outcome. Maybe something to consider or add to the list?
SWIM admires your desire to stop the cylce that surrounds crack. Its a difficult thing to do. good luck!
