Swim had some of his 10/325's left over from his surgery and took 12 or so of them last night. Swim feels alright just a huge head ache and a fever. Swim thinks this is normal and just being impatient, should he take one today to make withdrawals less? Swim knows he was close to APAP toxicity. Any suggestions?

I hope this 3.9 grams of acetaminophen was over a 24 hour period, if not then i would advice against taking any more. If this was a one off situation and not the end of a binge then i really can't see how withdrawal symptoms would be an issue.

If the 12 were taken over 12 - 24 hours then it would be OK to take one more at least 6 hours after the last dose as long as that dose was no more than 1000mg of acetaminophen, depending on the last dose i would give it about 8 - 10 hours at least. If the 12 were taken within a short period of time (please don't tell me you took them all at once) then i think a period of at least 12 - 24 hours before taking any more would be best. If they were taken all at once (please tell me you didn't do this!) then you might have problems because taking a 24 hour dose of acetaminophen in one go is just crazy and your symptoms may well be due to toxic effects, if that's the case i would seek medical attention.

Why was a cold water extraction not done on these tablets?

Swim took about 4 to start, 3 more about an hour later, and one every hour for about 4 more hours. Why no CWE? Good question, Swim not being too intelligent. Swim is feeling bloated a bit. He has been drinking water for a while and eating regularly. What would medical attention do for swim?

Swim called his local poison control office and they collected some information and came up with a conclusion that he would be alright. Though they seemed more worried about him being suicidal then the APAP. Swim still has a headache and a little clammy.

A Norco would take that headache away. Only one is needed for that. You should be fine with the amount of tylenol swiy took especially if you're not a habitual user.

A 24 hour dose in about 5 hours, that is really bad man and i probably would have suggested medical attention but i think the right thing was done by phoning poison control, good thinking, i wouldn't have thought of that (probably because i am unaware of anything like that in the UK). It should be all right but if there are any changes and symptoms do not improve or get worse then maybe think about getting checked out. IF there is any sign of yellowness in the eyes or skin (it will probably appear in the whites of the eye first) then you must get treatment immediately because it's a sign of liver failure.

I don't want to scare any one and the right thing was done by phoning poison control and if they say it will be all right then it should be fine.

Next time a cold water extraction is a must when taking a recreational dose of something that will be over the recommended dose of paracetamol/acetaminophen (or aspirin, ibuprofen etc). Paracetamol is just too dangerous to play about with, although there is probably some margin of error in the recommended dose, personally i would never take more than 1500mg at any one time and 5000mg in a 24 hour period, anything more and it can and does become dangerous. To be honest i probably wouldn't even take 1500mg at one time (3 standard paracetamol tablets here in the UK), if i had to take more than 2 for what ever reason, then a cold water extraction would be the best thing to do. I wouldn't take more than 4000mg in a 24 hour period either, i was really just giving an idea of what would probably be safe. I have to stress this 24 hour thing because some people think it's 4000mg in a day and end up taking more than the recommended dose, sometimes people can do this for days in a row causing build up of paracetamol which could cause problems.

Well, i am sure a lesson has been learned and it won't be done again any time soon, at least i hope so.

As for the withdrawal thing, was this the end of a binge that lasted a few days, or was it just a one time thing? If it was a one time thing and there are no other issues with taking these things for pain over a couple of weeks or more then there really shouldn't be any withdrawals. If the medication has been taken for some time for therapeutic reasons then the only thing i can suggest is to cold water extract even 2 - 4 pills. After 24 hours then it should be OK to take a couple of pills without any extraction, but i would leave it 24 hours at least, i am not a doctor but it's what i would do.
