Suboxone (buprenorphine/naloxone) for opiate overdose?

SWIM was thinking earlier, he sure does wish he had some Narcan (naloxone) around in case he had an opiate overdose. He could get a trip sitter to administer it in the event he took on a risky experiment. He was wondering where he might find this, and realized he already had some laying about! ... In the form of Suboxone (buprenorphine/naloxone). He was wondering, in the event of an opiate overdose, would IV-ing a Suboxone (8mg bupe/2mg naloxone) effectively reverse it? SWIM knows that you'd be adding a small amount of an agonist/antagonist that may cause a small increase in CNS depression, but then the naloxone would activate due to the IV administration, and reverse a large portion of the opiate agonism. He is curious if this has ever been done, because it sounds like a promising source for life-saving naloxone thats available on the street for many SWIMers. For this SWIM, if he has overdosed anyway, 8mg of bupe is fairly negligable, especially if the majority of the CNS depression is effectively negated by the naloxone.

Thoughts? Dreams? Aspirations?

From what Swibf remembers bupe is 2-3x the potency of methadone. Can't say whether the combination of the two would be beneficial or not.

Buprenorphine more powerful than methadone?Maybe for binding to receptors certainly not for euphoria.

Buprenorphine is only a partial agonist and antagonist so no it won't have the same euphoric potential.

"It should be noted that, in an effort to prevent injection of the drug, the Suboxone formulation includes naloxone in addition to the buprenorphine. When naloxone is injected, it is supposed to precipitate opiate withdrawal and blocks the effects of any opiate. However, contrary to popular belief, since buprenorphine has such a high affinity (higher then naloxone), the naloxone does not affect the user even when injected. The naloxone does not precipitate withdrawal or block the effect of the buprenorphine when taken sublingually."

I know this is an old thread, but I have a relevant response that answers the OP.

Absolutely suboxone can be used in a pinch to reverse opiate overdose. But not just because of the naloxone.. The buprenorphine has such a high affinity for the receptors that it will knock off existing opiates.

Even administered sublingually, as long as you have a pulse, it should work within minutes. 8mg should be more than enough to pull out of overdose, but if that doesn't work 16mg will completely block the receptors. (Removing all opiates from the brain.. extremely painful in an addicted person)

However, not enough research has been performed in this area to justify NOT carrying naloxone, so you will not find any information advising it. However the link below seems to confirm my long-held theory.

There was a time a friend of mine I was helping from time to time try and get work and they had an experience and a half, literally they they had a half bundle a day habit and it was about 20 hours since last half done about 12 since last bag they forgot this sneak extra bag and it must have been fatter than they expected and wound up booting a Sub strip shortly much to everyones dismay after that around 3mg and this threw him into a rush and half, nearly on point of hives, extreme panic set in, huge pupils, he was tripping and had to get to the bathroom, the shower, water, he kept screaming STAY ALIVE! Jumping in the warm shower for about an hour was the only cure and when he got out felt much better, almost cured and cozy if you will in a very nice change of pace kind of way, free and much better, CLEAN! It was an incredible experience and wonders if anyone else has had similiar happen? He said at first he thought instantly 2 things heart attack or panic attack out of his worst nightmare kind of rush happened. So this would be an actual account of this occuring. They are still sober from full agonist opiates since then, a year ago to the date.
