Doing IV Heroin alone

The last time SWIM did IV heroin he was by himself, and must have OD'ed. SWIM came-to under his desk, with a couple of nasty grazes to the head, where he must have hit himself as he passed out. The previous day, SWIM has taken exactly the same dose from exactly the same batch without any problems.

Doing IV Heroin without anyone else around is probably not a great idea. I'd be interested to know how many SWIY have done IV by themselves. Is there any way it can be made safer?

None of SWIM's friends are into H, so shooting with someone else isn't really an option open to him.
