It must be difficult to ween off of heroin?

I'm making this assumption because I have never seen or read anything concerning this. I hear about people switching from heroin to other drugs, then weening off of the new drug, but why not ween off heroin itself, just reducing the dosage over a period of time? One reason I personally suspect is the fact that the potency of heroin can very quite allot, which would make it hard to accurately decrease the doses?
