An abandoned maintenance opioid related to methadone.

Re: What is ORLAAM?

ORLAAM is a discontinued product that was used, as you said, in maintenance programs, usually after methadone had failed. It contained the synthetic opioid levomethadyl acetate. It was discontinued after some serious heart related problems amongst patients.

EDIT: FFS I need to learn to type faster. Posted at same time as above.

Re: What is ORLAAM?

Thanks, swim knew it must not be popular if he's never even seen it mentioned here.It says a person only needed to take it three times a week. That sounds pretty cool.Ofcourse it also said it was related to some severe cardiac adverse related events which does not sound cool.Swim would definitely try to give it a go if it were still around.Probably better than buprenorphine.

Re: What is ORLAAM?

Quote:
Methadone begins to work faster than levomethadyl does when treatment is started, but it has to be taken every day. Levomethadyl starts to work slowly, but it does not have to be taken every day. Therefore, some people may start treatment with methadone and then change over to levomethadyl after several weeks. Other people may receive only one medicine or the other during the entire time of treatment.
may be helpful as all of the other information is already stated above.

the FDA says
Quote:
Transfer from ORLAAM® to Methadone:
Patients maintained on ORLAAM may be transferred directly to methadone. Because of the difference between the two compounds’ metabolites and their pharmacological half-lives, it is recommended that methadone be started on a daily dose at 80% of the ORLAAM dose being replaced; the initial methadone dose must be given no sooner than 48 hours after the last ORLAAM dose. Subsequent increases or decreases of 5 to 10 mg in the daily methadone dose may be given to control symptoms of withdrawal or, less likely, symptoms of excessive sedation, in accordance with clinical observations.

hope this has been of help

I agree, buprenorphine never did agree with swim. But unfortunately Orlaam was discontinued in 2003 in the U.S. due to ventricular rhythm disorders in some people.
