swim just got 60 fentanyl patch 75mch/h 72hour patch and swim does not want to smoke the gel swim wants advicein how to best enjoy these without smoking?

Simply stick the patch on your skin to be safe. If this isn't doing it for you, you could try putting two on, but be careful.

Swim has removed a bit of the gel, mixed with water and shot it a few times. This is an idiotic thing to do, and I wouldn't recommend that any reasonable person try it. Some of the times Swim felt almost nothing at all, whereas other times Swim was almost knocked out by the power of the shot, it being almost too strong. It is difficult to measure how much drug is in the gel so it is safest to leave it in the patch and go with the transdermal route.

While simply wearing the patch on SWIYour skin will work fairly nicely, SWIM and many other fentanyl veterans find buccal/sublingual administration more satisfying.

Your description of the patch containing "gel" has SWIM assuming that SWIyou've got duragesic patches, or their generics. To try using the fentanyl sublingually, SWIM suggests SWIyou do the following:

- If SWIyou've got no tolerance to opioids/opiates, SWIyou should start his dosing with no more than approximately one-tenth of the gel contained in the patch.

- To extract the gel, SWIyou can either poke a hole in the patch's resevoir with a pin, or cut one of the corners so that an opening about the size of a matchhead is left. SWIM definitely reccomends cutting the corner as opposed to poking a hole.

- Once an opening is made, SWIyou must be sure to always keep the now-opened patch stored in an air-tight container.

- To administer the fentanyl, SWIyou would squeeze out about 1/15 to 1/10 of the patch's gel under his tongue or on his lower lip between the lip and gums. It's very important that SWIyou avoid swallowing any of the gel, as it is best absorbed in the lining of the mouth.

- The drug's effects should be noticed within a few minutes. Be sure to wait at least half an hour before dosing again. The initial amount should be plenty for SWIyou, but he can titrate up if necassary.

***Remember that fentanyl is an extremely powerful and potent opioid. It's a killer, literally. Be absolutely sure to read as much as possible about the drug before using it.
