This is the hardest part for any swim trying to get clean, once the opiate queen has her claws in you it is very hard to stop. My advice would be to gradually reduce yourself to the point where swiy can cope without the need for any opiates, it also entails finding new things to fill your time with. Swim doesn't see a problem with contuing smoking dope as this may help ease swiys cravings. Good luck on staying clean, I hope you can do it.

Cutting out the heroin would probably be the biggest issue, or at least reducing it to occasional intake. Daily use of opioids will lead to addiction quickly, as will benzos. But as stated above, only SWIY can decide when to stop, nobody can really make SWIY want to quit. SWIM would advise to try to cut benzo and ephedrine use to once a week, and opioid use once a month, and from there on work on reducing the weed, if SWIY wants.
