going to a doctor: will they record SWIM uses cocaine?

Say a person has used coke a couple of times and has a sinus infection.
If they go to the doctor and tell him what they did, will the doctor record that the person is a drug user? Even if they Never use it again?

Theres a thing called docter patient confidentiality. Basically, everything you tell your doctor, it's illegal for him to say to the police, even if you are a drug user. Even if you did get caught in any way, the doctor could have their licence removed.

No, the doctor won't turn you into the police. Yes, the doctor will note in your medical record that you are a drug user... and that will influence the way that medical group will handle you, deal with your symptoms, and prescribe medications to you in the future.

In most of the cases the remarks made before me are true. However there are some doctors that do not uphold the confidentiallity in drug cases.
I remember a case of someone dying after the ambulance driver dropped a intoxicated patient off at the police station. The patient was in a critical state.

If you are pregnant and a doctor finds drugs in your system - he/she is obliged to report you to the police and state department of social services (USA). Hospitals are required to test for drugs in pregnant patients. Then all hell will break loose. Children seized. You jailed to protect your unborn child. So forth.
