I remember about ten or fifteen years ago there were quite a few deaths due to heroin in Vancouver. Not that it was 'bad' heroin, it was extremely potent heroin. When the batch hit the streets a lot of junkies died on their first shot after scoring. When they didn't know it was extremely potent they just put the usual amount in the spoon, cooked it up, then OD'd.
