cocktail by the doctor not functioning: What to ask for?

Swim attempted suicide back in May. After a week long stay in rehab, he was released with a scrip for sertraline (50mg). After two months, swim was still having serious depression and anxiety issues. Went back to the PDOC, and was told to take 100mg of sertraline as well as .5mg/6hrs clonazepam. After a 2 weeks, swim went back to the PDOC and was switched from sertaline to 10mg of escitalopram and 1mg/6hrs clonazepam. After yet another 2 weeks, the dosage was increased to 20mg plus the clonazepam. For some reason swim's doctor didn't care to listen to swim about the effects being dealt with and the problems the situation was causing. On the next visit, the PDOC added 150mg of bupropion XL to the cocktail. After the next visit, that was upped to 300mg. Pissed off, swim took the scrip, tore it up and told that PDOC to go fuck himself.

Swim got a new PDOC, and she agreed somewhat with the previous diagnosis, but added trazadone for insomnia and recommended swim for neurological testing for ADD and talked with me about adderall and ritalin.

Now to the point, swim is taking 100mg a night of trazadone and CANNOT sleep untill exhaustion gives in at about 24-36hrs of sheer tired, cracked out feeling.

Swim's next visit with the PDOC is next friday. She plans to reduce the lexapro and clonazepam until swim is off them. What should swim ask for, for the insomnia. Swim was under the impression trazadone was pretty powerful, but it has literally no effect.

Help, please, before swim jumps infront of a bus!
