Help with Doctor please (back pain)

Swim needs Swiy opinion!

Know it has been covert a few times on the forum but swim has conflicting feelings on if it is wise to puch the boat out.

This is swims situation:
He is 29 years old.
Having lower back pain for about 4 months.
Told doctor that he tryed Paramol and Nuroven plus.
Doctor given him Diclofenac Sodium (told doc they made him feel sick)
Doctor given him Co Codamol 30-500 (after 2 weeks phoned receptionist asked for something better, She told swim that he was on the strongest one and doubt he would get anything else, But swim got Tramadol)

Now the thing is swim wants to go for something stronger!
Swim told the nurse in a other surgery that he has been addicted to many drugs heroine, coke, meth and taken oxazepam, diazepam, thermazepam etc.
And that he now only smokes pot sometimes.

Swim not sure if the file has moved to his current surgery (been there for 2 years now ) This surgery didn't ask swim if he ever had taken anything and doctor never sad anything.

Swim told his doc on his last visit, That his grandad (fathers side) was signed of work for life when he was 45 due to his bad back, That his grandma (mothers side) and his mum has bad backs and that his dad is a heroine addict so even if he had a bad back he wouldn't feel it. All of it is true!

Swim told doc when on holiday in Thailand he got something strong in a pharmacy and it worked, But didn't know what it was. (is a lie he never got any pain killers but could have done) his doc wasn't negative abou it!

Swims doc is known te be easy with dishing out medicines. Many think he isn't a good doc. But swim likes him!!

His doc given him in this order: zyban (stop smoking) Then swim put in repeat prescription for zyban after starting smoking again. Pharmacist had a note from surgery tellin swim of.But got them.
Then swim got a beta blocker (propranolol) for panic attacks. Swim asked it to be discontinued after 4 weeks even doc told him he can get more if he needs them again.
Then he asked for champix (stop smoking) He got them without a problem. Swim has started smoking again. He got 14x2mg diazepam (valium) for flying.
Then for his back see the start of this tread.
He is now on Tramadol, Had: 2 times: 60x50mg.

Whats the best way to approach this ?

Should swim tell him he tryed Diyhydrocodeine and it worked ?
Let the doc decide ?
Tell the doc he found the emty box in his suithcase and that it was Diyhydrocodeine (slow release), Hydrocodone, Buprenorphine etc. that he got in Thailand.

Swim can argue if doc says that its open to ubuse or thinks swim isn't genuine. Swim would tell him if he wanted to abuse he will go back and live in Holland or he could get them online. And that the docs now pushing people in to self medicating etc.

If needed swim can change docs as there ones closer by swims home.

Sorry to go on but swim want acurate info!
Have read other great treads on this specially by Richard smoker.
But want opinions on swims personal sutuation.
Swim thinks he should tell his doc what to give and push the boat out!

you have told too much info to your current Dr. , No way will he prescribe any opiates to someone who is prone to addiction.

In my opinion.

If you do change doctor, then do not mention any of those addictions, or family members addiction, they are red flags.

Wait, why did you tell them that you had a history/current drug abuse?

I think it depends on the docter, legally I obtained ambien from my psiciatrist after she knew of swims *past* drug habbits (he told her he smoked pot currently and she was still down)

Just my personal opinion here...

Do not ask for specific drugs. If you've taken something in the past and it's worked for you then you can say "I've taken such and such and it's worked for me in the past" but do not ask for a specific script. As strange as it sounds doctors need to feel like they're calling the shots. Allow the doctor to perscribe what he feels is best. Try it and give it some time. If it's not working, be honest and tell the doc it's not working. Let the doctor follow the protocols. From what I understand, this is most important in the UK.

As far as past abuse goes, knowledge of it will most likely cause the doctor to be more cautious. Since you've already been down that road, you may want to suggest close monitoring, periodic drug testing, etc. in order to make the doctor more confident about perscribing you drugs with a high abuse potential.

Thanks to all, Swim didn't say anything to his curren't Gp and he never mentioned it. He told the practice nurse at his old surgery about 15 miles away and she put it in the computer. Swim knows what a idiot he is and will never say anything again!!

Swim works at a closed mental unit for the elderly and has a big dog (presa canario google it) He was a assistand bar manager before and his family have bad backs. His Doctor knows this. And work like swim is doing and back pain go hand in hand.

Swim is thanking everyone for advice and will get 60 Tramadol a month instead of 60 every 2 months. For the next few months and then get 60 every 2-3 weeks. Then swim will tell his Doctor the Tramadol isn't working as good anymore, And see what he gets next.

Swim gets 60 Tramadol and swim can take up 8 a day. So 60 in 2 months or 60 a month doesn't look like abuse to swim.

If any of you swiy ever end up in a mental unit remember the bigger the pain some one is the more tablets they get!! Annoy the nursing staff, be aggresive, Be angry etc. And they will ask the Doctor for something to keep you quiet.

I dont knowabout that, back in 7th grade swim was on Ritalen(sp) and he straight asked a psiciatrist to go on Adderall, and he hooked it.
