Best Opiate for long ter clinical use

Swim has a nasty long term case of Cronic Pelvic Pain Syndrome caused by a dodgy prostate gland. Swim has really had a difficult time over the last few years due to this affliction. Swim was prescribed Trammadol and this was extremely affective for 4 months but has built up a resistance and is seeking an alternative drug to Trammadol. A GP (not my regular one) said " it's trammadol or morphine" (English isn't his native tounge) but swim is sure there is a "middle ground" medication between these two. ( Tram to morphine seems like a massive jump to me) Swim is seeing a pain specialist in the next couple of weeks and would like to have some alterative medications to discuss. Any ideas?
