Suboxone Q
Can SWIM get a difinative answer on having to be in withdrawl before using Suboxone? Is it a must? SWIM hears that it will put SWIY into withdrawl in which there is no turning back. So that would mean SWIM needs to wait a few days until he's feeling shitty before dosing?

These aren't my words but taken from a suboxone information site made by the makers of suboxone:
"It is important to be in mild-to-moderate withdrawal when you take your first dose of SUBOXONE. If you have high levels of another opioid in your system, SUBOXONE will compete with the other opioid molecules and knock them off the receptors. SUBOXONE then replaces those opioid molecules on the receptors, but because SUBOXONE has less opioid effects than full opioid agonists, you may go into withdrawal and feel sick" There you go. |

Swim took this in a residential detox. He did his usual dope that day up until he entered treatment slept that night and the next morning when he usually got down to biz, he took his first dose of soboxone instead. Swim never felt a thing. Swim's suggestion is to take when swiy honestly starts to feel mild withdrawal. Runny nose, anxiety, nauseousness, whatever. Swim praises this method over the evil methadone so good choice. Unless someone say's otherwise and this is swims guess, a few days is way to long, more like 24 hours give or take, but again its all about swiy's honesty and when he first starts to feel sick.

There is no definitive answer for whether you have to be in widthdrawl before taking buprenorphine/suboxone. For some people, especially methadone patient switching to suboxone, taking buprenorphine when you are not in withdrawl can cause a precipiated withdrawl, worse than anything you would normally get. However this entirely depends on the person, their tolerance, and habit. SWIM has known lots of people who have not waited until withdrawl set in to take buprenorphine and most of them have been fine, meaning the buprenorphine did not cause any withdrawl. The main group of people that SWIM has heard this causing problems for is people who are already on methadone. SWIMs opinion is that waiting for withdrawl would be more important for people with larger habits and for people switching from methadone. Buprenorphine is very powerful and has a very strong affinity for your opiate receptors, however it is only a partial agonist.. so it will fight with opiates already in your brain and knock them off the receptors. Bascially, if the suboxone that is left over still has the power to keep your opiate receptors quiet then no big deal... But if buprenorphine knocks all the opiates off of your receptors and then turns out to not be strong enough to "hold down the fort", then you are in for some displeasure.

So SWIM waited till SWIM was feeling a nasty withdrawl from Methadone and OC. SWIM started taking vitamins, showering a lot, sleeping well, eating as healthy and as much as possible and taking Suboxone.
SWIM has the Suboxone 8's and took 1 yesterday morning and 1.5 today. They didnt help much yesterday and are not really helping today. SWIM is still felling really shitty. Are SWIMs withdrawls too strong for the Suboxone? SWIM dows not want to take more than 2 a day as he fears he will then w/d from the bup. SWIM will not be taking the Suboxone for more than a week. |

Oh man, if your coming off of methadone it's not going to be an easy transition. I've consistently read accounts of people switching from methadone to suboxone, and it seems to be the hardest. Generally it's not even recommended that you try to switch from methadone to suboxone until your dependancy has been tapered down to 30mg and you are comfortable there. I don't know what dosage of methadone/OC you were at when you let yourself go into withdrawls, but I'd say you might need to up your suboxone dosage a good bit.
These long-acting opiods put your tolerance through the roof... they are like having a monkey on your back, but one that is sleeping most of the time. Short-acting opiates is like having a monkey on your back but he's always awake. |

SWIM has been on Suboxone for about 6 months and used dope (heroin) for a little over a week and stopped. Then SWIM took suboxone the next day when in very mild withdrawl. Thirty minutes after taking suboxone he went into very bad withdrawl started getting very hot/cold, sweats, aches, ect. SWIM took some the next day and didn't improve. Then took some the next day and still didn't improve but withdrawl got better. Finally day four suboxone worked. To get to the point SWIM highly suggest SWIY get into moderate withdrawl and feel pretty bad until SWIY cannot take it anymore then take it

As soon as SWIY feel the first signs of wd dose the Suboxone, 12 to 24 hrs after last opioid use. SWIM has found that to be the most effective way. His was methadone detox and it worked very well for making him mentaly feel better but didn't take away the body pains.
